Required Documents - Graduate

Here are the documents required to complete your application(s). While transcripts must come directly from your school, you can submit the rest through the myOzTREKK portal.

Send Official Transcripts

❑ OzTREKK requires one copy of your official transcript from each institution at which you’ve studied.

Important: Transcripts must be up to date and in English.

Email: Please have your school/admissions office email us directly or through an official transcript service (e.g., Parchment or MyCreds) to transcripts@oztrekk.com. If you require a password on the digital transcript, please use the following:

  • Question: The educational agent that I am working with
  • Password: OZTREKK1 (no spaces, all capitals) or if there’s a character limit or an access code requirement, use 1234

Mail: If they cannot be emailed, transcripts in their original, sealed envelopes can be mailed to the OzTREKK Office.

Send Interim Transcripts
You can start an application while your undergraduate degree is still in progress. For OzTREKK to submit your application to the university, you must be in your final semester and your interim transcript must show your enrolment in all courses required to graduate.

Step 2

Send Passport Photo Page & Complete SSVF

❑  Copy of your passport photo page; must be in colour (PDF) – Please upload to the myOzTREKK portal.
❑  Fill out the online OzTREKK SSVF Form – We collect this information to meet compliance requirements. If we require further information, we may contact you.

Step 3

Send University-specific Documents

Please upload your documents to the myOzTREKK portal. We only require one copy of each document, even if you’re applying to multiple programs.

If a University Requires Course Outlines
Course outlines demonstrate your knowledge of prerequisite subjects. Universities require detailed course outlines that show all covered topics and include a lecture schedule. One-paragraph course descriptions will not suffice. If you don’t have these, please let us know as we may have copies in our files. If we don’t, please contact the faculty from which you undertook your studies.

 

Adelaide University – Master of Physiotherapy

No additional documents required. If you’ve completed steps 1 and 2, above, your application is ready to submit.

❑   AUD$150 application fee paid directly to the university (link will be provided). Please let us know if you do not wish to pay the application fee and we will cancel your app immediately.

 

Bond University – Doctor of Physiotherapy

❑   Résumé
❑   Prerequisite course outlines (Please check pre-approved Canadian courses. Course outlines are not required if preapproved.)

  • Human anatomy or equivalent (2 semesters)
  • Human physiology/exercise physiology/exercise prescription or equivalent (2 semesters)
  • Research design/statistics or equivalent (1 semester)

 

Curtin University – Doctor of Physiotherapy

❑   Prerequisite Self-assessment Form
❑   Prerequisite course outlines

  • human musculoskeletal anatomy (1 semester) (bones, joints, and muscles; cannot be an online unit and must include wet lab/cadaveric content)
  • human biology or physiology (1 semester)
  • health research methods or evidence-based practice (1 semester) (desirable units include physics, psychology, and statistics)

❑   Video statement – Applicants are required to create a personal statement video in which they introduce themselves and reflect on their experience with physiotherapy and musculoskeletal limb anatomy:

  • Maximum 3 minutes
  • Clear audio and picture (must be able to see your face clearly)
  • Video storage platforms include and are not limited to Google Drive, OneDrive, Dropbox, YouTube, WeTransfer etc.
  • Link must not expire or require password

 

Flinders University – Master of Physiotherapy

❑   Self-assessment Form (See the sample form here)
❑   Prerequisite course outlines

  • Human anatomy (cell/systems and musculoskeletal) (2 semesters)
  • Human physiology (cell, systems) (1 semester)
  • Exercise physiology (1 semester)
  • Psychology/sociology (1 semester)
  • Research methods (1 semester)

Flinders requires true research methods (statistics does not meet this requirement).

 

La Trobe University – Master of Physiotherapy Practice

  • musculoskeletal anatomy (2 semesters) (includes upper/lower limb, trunk, head/neck, and neuroanatomy)
  • human physiology (1 semester)

❑   Résumé – Only if your completed degree is more than 5 years old.

OzTREKK note: We will automatically choose Bundoora Campus (suburb of Melbourne); however, if you would prefer Bendigo (150 km northwest of Melbourne) please let us know.

 

Macquarie University – Doctor of Physiotherapy

❑   Declaration Form
❑   Official high school transcript – (see “Send Official Transcripts” instructions above)
❑   You’ll require an application ID number to register for Casper. Please let us know which round you plan to sit as soon as possible, so we can send it to you.

 

Monash University – Doctor of Physiotherapy

❑   Prerequisite course outlines

  • human anatomy (1 semester)
  • human physiology (1 semester)
  • biostatistics (1 semester)

Your bachelor’s degree must include a minimum of 2 of the 3 prerequisite subject areas listed. Only 1 may be considered if it has been acquired through non-award or open study.

University of Melbourne – Doctor of Physiotherapy

❑   Download and fill out the University of Melbourne Agent Nomination Form (PDF) – Typed signatures are not accepted. Must include a handwritten or digital signature.
❑   Confirm you meet the prerequisites: human anatomy (1 semester) and human physiology (1 semester)

  • Review the “List of assessed subjects” on Melbourne’s prerequisites page.
  • Under “List of assessed subjects” heading, select the Doctor of Physiotherapy dropdown and choose “Assessed Subjects – International institutions”
  • Find your Canadian university and ensure it has “approved” beside the courses you’ve taken.

❑   Résumé (if your degree was completed more than 2 years from the DPT program start date)

Melbourne is in the process of replacing their multi-mini interview (MMI) with the Casper situational judgement test. This SJT will not be required as part of the initial application process but instead after shortlisting has occurred.

 

University of Notre Dame Australia – Master of Physiotherapy

  • pathophysiology (1 semester)

Applicants are required to meet Notre Dame’s human pathophysiology requirement. Should students not meet this prerequisite but meet the other prerequisites and GPA requirements, the university offers a free, self-paced module, which will be a condition for completion to an offer for a place in the program.

 

University of Queensland – Master of Physiotherapy Studies
