Documents Required

Find out which documents are needed to complete your public health school application. Some documents you can complete online, and some you'll need to send us as PDFs. We break the process down for you in 3 easy steps!

Step 1

Send Official Transcripts

OzTREKK requires one copy of your official transcript, from all institutions at which you’ve studied (must be in English). Please send transcripts via

Email (Preferred): Transcripts must be sent from the school office/admissions office directly or through an official transcript service (e.g., Parchment or MyCreds) to transcripts@oztrekk.com. If you need to have a password on the digital transcript, please use the following:

  • Question: The educational agent that I am working with
  • Password: OZTREKK1 (no spaces, all capitals) or if there’s a character limit or an access code requirement, use 1234

Mail: If they cannot be emailed, transcripts in their original, sealed envelopes must be mailed to the OzTREKK Office.

Step 2

Send Passport Photo Page, CV & Complete SSVF

❑  Scan of your passport photo page (PDF) – Please send to rehabsci@oztrekk.com.
❑  Fill out the online OzTREKK SSVF Form – We collect this information to meet compliance requirements. If we require further information, we may contact you.
  Résumé/ CV – Include working history if applicable and should also specify if the positions were part time or full time and the number of years/months employed. Please send to rehabsci@oztrekk.com.

Step 3

Send University-specific Documents

OzTREKK only requires one copy of each document, even if you’re applying to multiple programs. All PDFs must be submitted to rehabsci@oztrekk.com. We’ll confirm receipt of your documents, so you can keep track.

 

Deakin University – Master of Public Health

Flinders University – Master of Public Health

No additional documents required. If you’ve completed steps 1 and 2, above, your application is ready to submit.

Griffith University – Master of Public Health or Master of Global Health

   Select a major focus of study: dissertation, standard, or undeclared

Important to remember:

James Cook University – Master of Public Health

   JCU-specific personal statement (100–250 words outlining your interest in JCU’s program)
  JCU-specific Application Form – You must provide a contact for one of Facebook Messenger, Viber, WhatsApp or WeChat; mandatory for all students.

La Trobe University – Master of Public Health

   Complete a personal statement explaining why you’re interested in the La Trobe MPH program.

 

Macquarie University – Master of Public Health

No additional documents required. If you’ve completed steps 1 and 2, above, your application is ready to submit.

Important to remember:

  • Rolling admissions; apply as early as possible. Deadline is mid-January for February intake and mid-July for end of July intake.

 

University of Melbourne – Master of Public Health

   Download and fill out the University of Melbourne Agent Nomination Form (PDF) – Typed signatures are not accepted. Must include a handwritten or digital signature.
   Melbourne-specific personal statement (250 words outlining why you wish to enroll)
  Melbourne MPH Referee Report (optional) – Please include if you have 2 years + of full-time paid work in a public health field and would like to be considered for advanced standing based on this work experience. It is recommended that you provide one report for each role/position held in that field, as well as a detailed CV including working history, specifying if the positions were part time or full time and the number of years/months employed. You must inform OzTREKK if you plan to include these documents.
   A CV (Optional) – Only required for applicants with borderline results but who have substantial relevant work experience.

Important to remember:

  • Deadline is typically the end of October for February intake or end of June for July intake; however, you are encouraged to apply at least 6 months prior to the program start date.

 

Monash University – Master of Public Health

   Monash-specific personal statement of purpose (250 words outlining why you wish to enroll)
   Reference letters – Must include dates of employment, basis of employment (full/part time), position title held, and list of responsibilities/specific duties; signed and dated by the reference and printed on the company letterhead paper. Must be sent directly from the referee to rehabsci@oztrekk.com or by mail to OzTREKK.
   Résumé/CV – Must have relevant experience in health or public health

  • Rolling admissions; apply as early as possible.
  • AUD$100 Monash application fee paid directly to the university (link will be provided). Please let us know if you do not wish to pay the application fee and we will cancel your app immediately.

 

University of Queensland – Master of Public Health

   Review UQ’s Transfer Policy and Procedures
   Official high school transcript (see “Send Official Transcripts” instructions above)

Important to remember:

  • Rolling admissions; apply as early as possible. Deadline is typically the end of November for February intake or end of May for July intake.
  • Must have a health-related undergraduate degree or one year (full-time) experience in healthcare setting.

 

University of Sydney – Master of Public Health or Master of Global Health

Master of Global Health
   Résumé/CV must have relevant global/public health work experience
   Statement of intent (your motivation for studying the course)

Master of Public Health
   Résumé/CV must have relevant public health work experience

Important to remember:

  • Rolling admissions; apply as early as possible. Deadline is end of January for February intake or end of June for August intake
  • AUD$150 Sydney application fee paid directly to the university (link will be provided). Please let us know if you do not wish to pay the fee and we’ll cancel your application.

 

University of Western Australia – Master of Public Health

No additional documents required. If you’ve completed steps 1 and 2, above, your application is ready to submit.
